The Series C transition is the highest-volume trigger
Around Series C, most biotech boards revisit whether the founder-CEO is the right person to take the company through pivotal trials, approval and launch. This is not a failure verdict on the founder; it is a recognition that the CEO profile for a discovery-and-development company is not always the CEO profile for a commercial-transition company. This transition is the single most common driver of biotech CEO search we see in H2 2026.
CEO compensation has repriced
Clinical-stage biotech CEO compensation in the UK has moved up materially in the last twenty-four months, particularly on the equity component. Where a Series B UK biotech CEO would previously have accepted a 2-3% founder-equivalent equity ratchet, the market benchmark is now 3-5% at Series C entry and higher for exceptional profiles. Cash compensation has moved less sharply.
The clinical-stage vs commercial-stage profile split is now sharper than ever
The biggest cause of biotech CEO failure remains placing a commercial-stage CEO into a clinical-stage role (or vice versa). The clinical-stage CEO profile — scientific and regulatory depth, capital-markets fluency, milestone-driven risk tolerance — has narrowed further in the last two years as boards have professionalised the definition. Commercial-stage profiles have widened to include more diverse sector backgrounds. This is a good thing for the market; it makes brief-writing more consequential.
CMO and Head of Regulatory scarcity
The Chief Medical Officer and Head of Regulatory Affairs pools are the most constrained in UK life sciences. Both roles have expanded materially in scope over the last five years — CMOs now typically hold commercial and payer strategy in addition to clinical; Heads of Regulatory now hold pathway strategy in addition to submission execution. The candidate pools have not expanded at the same pace.
